As part of some reconstructive foot surgeries for equino varus in November 2000, I had my Achillo tendon, the posterior tibialis tendon and DFT lengthened. I wore casts for several months then started with an aircast in January 2001. I had PT at home and then went to outpatient PT in February. Since March, when I had a third surgery for claw toes, I have been walking and doing aggressive PT. However, my calf in that leg remains very thin, no matter what I do. It was NOT as thin before the surgery and I wonder whether I will regain the bulk. I am 32 and I am curious about what causes the thin calf in people with equino varus. Could it be the lengthenings? I know people who wore leg casts for a long time and regained the muscle mass. Just want to know what lay ahead. Thanks.

When the achilles is lengthened, it is narrowed. Therefore, it will be thinner. Also, since you were in a cast for a long time, your muscle mass has decreased. Therefore, it will be smaller but you should be able to gradually increase your muscle mass. Try not to be too aggressive however so you do not get injured.
