Acute severe pain under big toe/ball of foot

I hope someone can provide some input. One week ago at night in bed, I suddenly had acute pain in my big toe near the bottom. If I moved the toe up or down I had excruciating pain. No swelling but warm to the touch. It was worse the next morning. I thoght maybe it just was sore so I very lightly massaged under the toe and ball of foot. It is worse. I went to my doctors and since gout runs in my family, the doctor did a uric acid test for gout which came back normal.
It's been a week and the pain is worse. It hurts even when I am laying in bed not moving with my foot elevated. Ice does not help, anti inflammatories don't help.
There is a constant dull pain with some burning and throbbing. I can't move the toe up or down without severe pain. The joint is stiff.

I am a 48 year old woman. There was no injury to my foot and no history of foot problems. I am nor sure what to do.The pain is terrible! Thanks
