I'm 43 years old, male, and have been generally healthy. I used to run a lot but stopped for many years. last week i ran for the first time in several years- about 2 miles on an indoor running track. I also vaguely remember stumbling and perhaps twisting my foot in the past week. In any case I had no problems with my feet until 2 or 3 days after these events, and then I developed my problem. I'm not sure if the foot pain is related to the events or not.

The problem is that I now have exquisite pain- like a burning/ stabbing sensation- in my right foot. The pain is on the lateral plantar side of the foot, at a fairly specific area and inch or two from the heel and directly below the lateral malleolus at the plantar region. The pain doesn't occur when I walk normally, but comes on when I extend my knee and at the same time dorsally flex my foot or when I extend my knee and at the same time point the right foot medially. If the knee is bent, this pain is hardly elicited at all- the knee has to be extended.

The pain is excruciating when it is elicited and although I don't get it most of the time, it does come on with certain movements as I described.

What do I have? Should I manage this by doing nothing and waiting to see if it gets better- I've had it for 3 days now and it seems worse if anything. I'd appreciate a diagnosis and advice on management.

In order to be diagnosed properly, you need to see a podiatrist for a thorough examination. It sounds like you may have injured your peroneal tendons. Since the pain is getting worse, you should see a podiatrist as soon as possible. You should also try to limit your activity until you see a doctor.
