college athlete with severe sesamoid problems. help

I originally fractured my lateral sesamoid in july 2008 and came to my freshman year of college on crutches. After three months of recovery and cortisone shots i began to play. After the shots wore off the pain returned forcing me to the sidelines once again. I have since then been off of my foot and got a bone scan that read that I had fractured my medial sesamoid at some point and a small part of that has separated from the bone. The doctors are weary of surgery because i have had damage to both sesamoids and because they don't know what exactly is causing the pain, but I do not know what to do. any advice? I am afraid of ending my college career before it even begins.
