Sesamoiditis/Sesamoid Fractures

What are the best treatments for sesamoiditis? Is there a special type of physical therapist that I should go to? Does ultrasound treatment work? I am scheduled to get an orthotic made. If that doesn't help I will get a cortisone shot. Should I be using cold for the inflammation? I have been wearing a boot and will wear it for 5 weeks in total, though I am not sure it is helping much. Since anti inflammatory meds bother my stomach, is there anything that I can use right on the foot that might help? Any help would be greatly appreciated as this is a very disruptive injury!
