painful urination (not UTI or STD) HELP!

Since I was a small boy I've had "episodes" in which I urinate and it becomes very painful/uncomfortable when I stop. I feel like I need to keep going to make the pain stop, but I've got nothing left, empty. It happened quite a bit when I was as young as 8, then it seemed to get better during puberty. At about 19 it started happening worse than ever. It doesn't happen that often, but the pain is now unbearable. It only happens once, the next day everything feels better, like it never happened at all.
I've had ALL the tests done (my urologist is renowned for being an excellent doctor by the way). Urinalysis, STD test, ultrasounds, CAT scans, and I even had a scope done and NOTHING has ever given a clue as to what is going on. Everything is completely normal.
Well, it happened recently, and I managed to pee into a bottle, and to my amazement there was a bunch of small snowy white particles floating around on the bottom of the bottle. So, what is this stuff if it's not an infection?
Another thing, I was trying to bulk up so I was drinking a lot of protein shakes. The "episodes" seemed to occur very often when I was drinking that stuff. Could it be protein that's passing through my kidneys?? Could it be some obscure infection that flares up suddenly? Could it be semen mixing with my urine???
Does anybody have any ideas that my doc hasn't thought of? I'd sure appreciate the help, because this really, really hurts.
